Depending upon your insurance policy and carrier, you may be reimbursed partially or fully for any out of network services you receive from Dr. Zermeno. Dr. Zermeno will provide you with a Superbill upon request to submit to your insurance to seek reimbursement. It is best to contact your carrier for their reimbursement schedule before deciding to work with Dr. Zermeno.
Medicare:
Dr. Zermeno is not a Medicare Provider. Please be aware that Medicare will not reimburse it's enrollees for services provided by non-Medicare Providers. Patient's with Medicare will be asked to sign a Medicare Private Contract acknowledging their understanding of this.

For your convenience, Dr. Zermeno's office accepts payment by credit card, including HSA debit cards, check and cash.
Cancellation Policy:
Dr. Zermeno requires 24 business hours notice for cancellations. Therefore, cancellations for a Monday visit must be made early the Friday prior. Missed visits or late cancellations will be charged the full fee for the session.

In order to provide the highest quality of personalized attention to each patient, Dr. Zermeno is not contracted with any insurance carrier. Therefore, patients pay out of pocket for her services and are given the appropriate documentation to seek reimbursement from their insurance company.
